Domestic 1st class US mail: $5.00 per image

Foreign airmail: $10.00 per image

Federal Express: Billed to your account number

Licensing Agreement

By signing this form, I agree to the following:

Acceptance and compliance with the policies listed.
Item(s) will not be used for any purpose other than that stated on the permission form.
Credit will be given using the following citation: George A. Smathers Libraries, University of Florida.
Alteration of original image beyond standard cropping has been described on this form.
One copy of the publication/project shall be donated to the holding Collection at the University of Florida. In the case of film, a video copy is acceptable. Collections must be notified with the URL address of all website uses and notified of all URL address changes. Donation is requested, but not required for small personal publishing projects where no use fee is charged.
Use fees may be involved for using this material and I agree to pay any charges required by this request.
I understand and agree that as the owner of the materials, the George A. Smathers libraries gives no exclusive rights to any publisher or author, makes no warranties or representations, and assumes no responsibility whatsoever for any claims against those reproducing images or against the University of Florida, its employees, and its agents, by an author, artist, or photographer, his/her agent, estate, or any other part in connection with the reproduction of these materials.
I agree to indemnify the University of Florida, its employees and agents, and hold them harmless against any and all claims, including copyright infringement claims, royalty or fee demands, and/or other causes of actions, including attorney fees and all costs, arising as a result of the reproduction of the materials listed.
I understand that the licensing fee includes the $25 service fee required to cover internal processing costs and the cost to email, FTP, or create 1 DVD or other hard media copy with the digital files being licensed in TIFF or other available high resolution format. I understand that postage and handling, if images need to be mailed, will be an additional charge.
